Salary

  • £25,545 Basic / £29K - £32,500 per annum with on-call
  • Full Time, Permanent
  • Monday - Friday, 8:30 - 17:00
  • On-call one or two weeks per month @ £290 per week
  • On-call once per month: £29K
  • On-call twice per month: £32,500
  • On-Call Hours: Monday - Thursday: 17:00 - 22:00 Friday 17:00 - Monday morning

    Duties
    As a Care Coordinator, you will play a pivotal role in scheduling and managing care rotas for our dedicated team of homecare workers. Key responsibilities include:
  • Planning and organizing rotas to ensure continuity and quality of care
  • Managing day-to-day scheduling changes and ensuring all shifts are covered
  • Liaising with clients and their families to arrange and confirm care schedules
  • Monitoring care plans, logging changes, and updating our Staff Plan system
  • Providing support and guidance to the care team, responding to queries, and handling emergencies when on call
  • Ensuring all regulatory and compliance standards are met in every care plan and visit

    • Criteria
  • Minimum of 2 years’ experience in homecare.
  • Proven experience in care coordination and rota planning.
  • Car Driver Essential.
  • Strong organizational and prioritization skills.
  • Proficient with computer systems and scheduling software.
