Our services help people avoid hospital or accelerate them safely through it, with specialist support in their community or homes whenever needed. This role is to help deliver to NHS Diabetes Prevention Programme (NDPP) and NHS Type 2 Diabetes Path to Remission Programme (T2DR)

We are seeking a passionate and personable individual to establish and nurture relationships with our key stakeholders, including primary care networks, community groups, and local businesses. Your role will involve raising awareness of our services, encouraging referrals, and positioning the health & wellbeing team as a trusted healthcare provider and partner. This will include delivering presentations and leading promotional events.

Our health & wellbeing’s experienced team is dedicated to making the UK’s local communities healthier. We help people to understand and take control of their own health through sustained behaviour change, transforming lives for the better through weight management, quit smoking services, diabetes services, cardiovascular disease prevention and health coaching & social prescribing. We work with service users to make the changes that stick. Our people will be with you every step of the way, translating the science into healthy habits that add up to long term health improvement.

Every day you will...

  • Work alongside the Regional Manager to develop engagement action plans that address local key strategic priorities and raise the profile of our NDPP programme.
  • Attend GP practices, local health groups and primary care meetings to promote the service available in the area and develop, improve, and sustain referral pathways.
  • Report on progress against relevant KPI’s and targets, providing regular updates as requested on progress of local action plans.
  • Aid communications strategies (e.g. public open events, training and awareness raising sessions, communications materials, social media etc.), to promote the services and capture referrals.

    To thrive in this role you must have...

  • Experience working in a primary care setting or delivering NHS Diabetes Prevention Programme (NDPP) and NHS Type 2 Diabetes Path to Remission Programme (T2DR). 
  • A positive and proactive approach with the ability to motivate, engage and assist partnership delivery.
  • A passion for health improvement.
  • A UK driver’s license with access to a vehicle.
