Job Description Summary (Main duties and responsibilities)

The Diana Nurse will be responsible for supporting the management of a caseload of children, including those with complex health needs , technology dependent needs and children/ young people with life limiting/threatening illness. They will support coordination, assess, plan, implement and evaluate specialist care in the community setting.

  • Work in conjunction with both statutory and non-statutory agencies in providing packages of care for children referred to the service with support from senior colleagues when required
  • To plan and deliver care to prevent admission to hospital wherever possible and promote early discharge.
  • Demonstrate substantial application of skills and knowledge in Children’s Nursing in a variety of settings including community

Must be a car driver with ability to travel across Leicester Leicestershire and Rutland.
